New Drug Shows Promise in Preventing Arthritis and Diabetes

Recent research has revealed that an existing drug, originally designed for treating rheumatoid arthritis, may also help prevent the onset of autoimmune diseases such as arthritis and diabetes. This breakthrough could change the way these chronic conditions are addressed, offering hope to millions affected by these diseases.

Autoimmune diseases occur when the immune system mistakenly attacks the body’s own tissues. Conditions like rheumatoid arthritis lead to joint pain and inflammation, while type 1 diabetes results from the immune system destroying insulin-producing cells in the pancreas. With over one hundred types of autoimmune diseases affecting approximately one in ten people globally, the potential to prevent these conditions represents a significant advancement in medical science.

According to Professor Lucy Donaldson, director of research at Arthritis UK, the findings are encouraging. “We have the possibility that we could get in early and give something that would halt the progress, or at least slow it down,” she stated. This research is detailed in a study published in The Lancet Rheumatology.

Biologics: A New Approach to Autoimmunity

For years, treatment of autoimmune diseases relied on broad-spectrum medications such as steroids, which can suppress the immune system but also increase susceptibility to infections. The development of biologics has marked a turning point in treatment. These lab-made drugs target specific immune cells, providing a more refined approach to managing autoimmunity.

The study conducted by researchers focused on a biologic called abatacept, commonly used in rheumatoid arthritis treatment. The first phase of the trial demonstrated that one year of treatment with abatacept reduced the risk of developing arthritis in individuals with autoantibodies, a biomarker indicating early immune system activity against the body. In the placebo group, 29 percent progressed to arthritis, compared to just 6 percent in the group receiving abatacept injections.

The second phase revealed that even after treatment ceased, the protective effects of abatacept continued for an additional four years, suggesting lasting changes in the immune system mechanisms that trigger the disease. “It shows that if you identify people where the autoimmune disease is motoring, and you target it, you can actually attenuate or even switch it off,” said Professor Andrew Cope, a rheumatologist at King’s College London.

Progress in Preventing Type 1 Diabetes

The findings from the arthritis study are not isolated. Similar advancements have been made in preventing type 1 diabetes, primarily affecting children and young adults. A biologic known as teplizumab has been shown to delay the onset of diabetes by an average of two to three years. Although this may seem modest, it can significantly ease the management burden for young patients, according to Dr. Rachel Besser, a pediatrician at the University of Oxford.

Teplizumab received approval from the Medicines and Healthcare products Regulatory Agency (MHRA) in August and is currently under review for use within the NHS. Another biologic, baricitinib, is also being tested for its potential to delay diabetes progression.

Research efforts are also underway to identify autoantibodies that could signal the onset of inflammatory bowel disease (IBD). Professor Geert D’Haens from Stichting Amsterdam University Medical Center is leading a study aimed at developing biomarkers for early detection of this condition. “If we can develop biomarkers, we could diagnose long before symptoms develop,” he noted.

These advancements underscore the transformative potential of biologics in not only managing but also preventing autoimmune diseases. As researchers continue to explore the mechanisms behind these conditions, the hope is that the knowledge gained will lead to innovative treatments across various autoimmune disorders.
